These terms and conditions (“Terms”) between you and Preventive Health Care India, having its corporate office at Mahavir Enclav. New Delhi 110058. (“Health Mitra”) governs your membership with Health Mitra and your use of the membership issued by Health Mitra (“Preventive Healthcheck Plan”). The Terms shall be deemed to have been accepted by you whenever you use Health Mitra’s Plans and/or any of the packages issued by Health Mitra, or avail any of the health benefits arising there from. You are understood to have read and agree to abide by (on your own behalf and on behalf of your dependents/family) the Terms. You are requested not to use the Health Mitra’s Preventive Healthcheck Plan or any of the Packages issued by Health Mitra, or avail any of the health benefits arising therefrom, if any of the terms and conditions contained in the Terms is unacceptable to you.

Consult any doctor and attach the bill with this form and send it to us at our office address or send us the scan copy of the voucher and bill at [email protected]. Post verifying all the bills we will deposit the amount (maximum reimbursement per consultation will not exceed Rs500(Rupees Five Hundred only) in your bank account as mentioned by you in the doctor consultation voucher.

1. This voucher is valid for doctor consultation only. It cannot be used to reimburse any IPD, medicine or any other medical expense except consultation.
2. Maximum reimbursement will be Rs 500 (Five hundred only) per voucher.
3. This voucher is valid for primary member and his/her family only.

Call Health Mitra helpdesk and book your pathology test and avail upto 30% discount on your pathology test.

1. This Card is valid in Indian Healthare empanelled labs only.
2. Card can be used for availing discount in selected labs only,it cannot be used for any cashless facility.
3. This Card Validity 1 Year.

Collectively referred as the “Health Mitra –Thyrocare Healthcheck Package” shall be issued by Health Mitra in manner as may be determined by Health Mitra at its sole discretion.

You may redeem the Health Mitra –Thyrocare Healthcheck Package” only once in order to avail the Health Mitra –Thyrocare Healthcheck Package A either by itself or along with the Health Mitra –Thyrocare’s Package B. It is hereby clarified that you may avail of Health Mitra-Thyrocare’s Package A as well as Health Mitra –Thyrocare Healthcheck Package B only if you avail of both simultaneously. You shall not be entitled to avail the Health Mitra –Thyrocare’s Package’s B without availing the Health Mitra –Thyrocare Healthcheck Package A.

It is hereby clarified that you may redeem the Health Mitra- Thyrocare’s Premier Package, Health Mitra- Thyrocare’s Classic Package and the Health Mitra –Thyrocare Freedom Healthcheck Package either separately or simultaneously with each other within the stated timeframe and validity by Health Mitra
