CANCELLATION AND REFUND POLICY

This cancellation and refund policy (“Policy”) governs the cancellation of your membership with Health Mitra, having its corporate office at 606 Vishal Tower District Centre Janakpuri New Delhi 110058.(“Health Mitra”) and of the membership issued by Health Mitra (“Healthcare Package Membership”), as well as the refund of the fees paid by you towards purchase of such Health Mitra Plan (“Membership Fees”).

In the event that you wish to cancel the Healthcare Package Membership and your membership with Health Mitra, you shall be required to intimate the same to Health Mitra, by calling Health Mitra, inbound or by sending an email within 14 (fourteen) days of receipt of the Healthcare Package Membership by you. You shall further be required to return the Healthcare Package Membership and all documents and other items provided to you along with the Healthcare Package Membership (“Healthcare Package Membership Welcome Pack”) by mailing the aforesaid to Health Mitra C/O Cancellation Department at Mahavir Enclav. New Delhi 110058 on receipt of such intimation and the Healthcare Package Membership's Welcome Pack, Health Mitra shall verify the details of your membership and the request for cancellation placed by you. On Health Mitra satisfaction that your request is in accordance with this agreement, Health Mitra shall carry out the following (collectively, “Cancellation”):

Important Refund Note: » In case no contact is established with the member(s) by Health Mitra central retention team for membership cancellation request, the cancellation process will be put on hold till further contact can be established.
» Cancellation/ refund will be processed only upon receipt / return of the purchased membership kit (unused) by Health Mitra.
» In case there are multiple ongoing dialogues between Health Mitra and the member(s), there may be few instances wherein the
